Succession Planning Questionnaire, from Permanent Equity, asks an owner to list the knowledge, relationships, metrics, and monthly decisions that run through them, then score how much culture, pace, strategy, and quality of output depend on them. It also asks who on the leadership team would run the company if the owner were incapacitated, and where passwords, facility access, bank authority, and signing authority sit. The form says to revisit the answers every six months, or sooner if needed.
Owners can map where the business depends on them, name who would run it, and list password, bank, and signing gaps before a leadership change. Advisors can use the same prompts in a succession conversation.
